Information and Consent

This document is designed to inform you, a client of Spirit of Faith Adoptions, about our professional relationship. Our team consists of licensed counselors and social workers, adoptive parents, adoptees, a birth parent, a single mom, lawyers, and pastors.  We provide weekly prayer support for all hopeful waiting families using first names only, and you are welcome to join our Wednesday morning prayer time.  We will also provide information about adoption resources and appropriate referrals as needed or requested now and in the future.  Spirit of Faith Adoptions is available to your family at any time by reaching us at 419-843-5355. Kathryn Gorney, LPC, ( is a Licensed Professional Counselor and Certified Adoption Assessor.  She is your primary contact and support person throughout your adoption journey with Spirit of Faith Adoptions.  At any time, you may also contact our Director, Stacy Knox, LPC: when you have a question or a concern.

The orientation of staff of Spirit of Faith Adoptions is Christian and we hold to the sanctity of human life.  If any client is uncomfortable with any counseling practice, please tell your assessor.  No one intends to impose any of their personal values on clients.  Spirit of Faith Adoptions' staff and colleagues value each individual as equal before God regardless of their age, sex, sexual orientation, race, nationality, or religion.

The role of all licensed adoption assessors of Spirit of Faith Adoptions is to assist you through the adoption process including the home study and post placement services which are regulated by the agency and the state of Ohio. All of our adoption assessors have completed all state required Adoption Assessor Training and thus, they are certified to conduct home studies and post adoption reports in the state of Ohio.  They have also completed Bachelors and/or Masters level degrees in which professional licensure in Social Work or Counseling was obtained through the Ohio Counselor, Social Worker and Marriage and Family Therapist Board.

Information contained in your home study will be obtained from documentation provided by you to Spirit of Faith Adoptions. This will include all paperwork required by the State of Ohio, criminal background checks (BCI and FBI), a report from the Central Registry on Child Abuse and Neglect, confidential references, any pertinent information released by you and provided from doctors, mental health professionals and/or teachers, as well as personal interviews we conduct with you, your spouse, your children and other household members. A search of the National Sex Offender Registry will also be conducted by SOFA staff on your behalf as required by Ohio law. Information collected during the assessment process will be used for the purpose of conducting a thorough homestudy.  

Spirit of Faith requires a minimum of 2 meetings during the home study process. Generally, each meeting will be a minimum of 1-2 hours in duration and those meetings will be scheduled in advance. At least one meeting will occur in your home and all household members need to be present, and the Home Safety Audit will be conducted. Adoption Assessors are required to meet individually with each person included in your household (4 years of age and older). In the event that you are unable to keep an appointment, you must notify your assessor via phone call or text a minimum of 24 hours in advance.  

Our adoption assessors will submit the home study draft to the Assessor Supervisor, Kathryn Gorney, LPC for review within two weeks of the final meeting as long as all of the required paperwork has been received by that time.  This document and post placement reports are submitted electronically.  Spirit of Faith Adoptions' staff will not disclose any information about you to any source outside of Spirit of Faith Adoptions without your prior written consent. However, all licensed adoption assessors are mandated reports and are therefore required by law to report to proper authorities when it comes to their attention that a person may be harmed or has suffered physical, sexual, or emotional abuse or neglect.

To review a draft of your home study, please schedule a face to face appointment with Kathryn Gorney, LPC, Assessor Supervisor of Spirit of Faith Adoptions. After all applicable fees have been paid by you directly to Spirit of Faith Adoptions, you may request to transfer or share your homestudy with other agencies.  This process is initiated by contacting Ellie Hartford: 

We assure you that our services will be rendered in a professional manner consistent with accepted ethical standards.  Please do not invite staff of Spirit of Faith Adoptions to personal social gatherings, offer gifts, or relate to us in any way other than in the professional context. You will be best served if our relationship concentrates exclusively on supporting your family through your lifelong journey of adoption.

Our professional relationship will begin when you electronically sign this document.  You are in complete control and may end our professional relationship at any point.  Spirit of Faith Adoptions offers adoption services to families as well as resources and support referrals as needed before, during and after the adoption process.  We encourage you to discuss openly and freely with us any questions or concerns you have at any time you wish.  If at any time for any reason, you are dissatisfied with our services, please let your adoption assessor know.  If that person is not able to resolve your concerns, please contact our Director, Stacy Knox at 419-843-5535.
