21-22 Spring Sports Physical Appointment Request
We strongly encourage all student-athletes to have their sports physicals administered by their family doctors if possible, therefore, opening up appointment slots for those student-athletes who may not be able to get an appointment before the start of the spring season.

Please complete the following form if you would like an appointment for the February 9th or Februray 23th Spring Sports Physicals being offered at Moorestown High School and William Allen Middle School.  After you complete the form, an appointment time will be sent to you.  MHS Sports Physicals will start at 7:30 am and WAMS Sports Physicals will start at 8:35 am.  If there is an issue with your appointment time, please contact the Athletic Director, Shawn Counard (scounard@mtps.us).  

Students will report to the nurses office.
