2. If the one seeking appointment is a representative, please do provide the name of the document owner.
3. The approved appointment is Non transferable.
4. Please provide a google hosted e-mail address
5. If the requester is not the document owner or immediate family member, a maximum of only two (2) transactions is allowed.
6. Due to the high volume of requests from clients within Metro Manila, some e-mail confirmations may be received after office hours or a day before your preferred date of appointment. Please bear with us.
7. Please be kind to check your spam folders also in case no e-mail confirmation was received.
