 1) By cheque:
       a) A crossed cheque should be made payable to "Federation of Medical Societies of Hong Kong"
       
       b) A separate cheque should be issued for each course;
       
       c) Please write down your full name, mobile no. and the course code: C411 at the back of the cheque;
       
       d) Please mark "Application for C411" on the envelope; and
       
       e) Please send to 4/F, Duke of Windsor Social Service Building, 15 Hennessy Road, Wanchai, Hong Kong on or before the Registration Deadline (13 August 2024).

2) By Cash:
      Please submit the exact payment within the office hours from Monday to Friday, 10 am - 1:00 pm or 2:00pm - 6:00 pm. on or before the Deadline (13 August 2024).

3) Payment Deadline: Course fee (including the mailing of cheques) must be received not more than 3 working days after the Registration Deadline. Any application with payment thereafter will be cancelled without notification.

4) Confirmation by email:  The successful applicants will receive the confirmation email within 1 week before the commencement of the course.

Guidelines for Video Lectures
1) Participants will be allowed to watch the videos within 3 days.

2) Participants have a maximum of 3 times to login and access each video link.

3) Participants are required to login and logout the system to collect the attendance record.

4) Participants can submit the questions in Q&A box after watching the videos.

5) Participants who have achieved 70% (4 out of 6 lectures) or above attendance will be entitled to an e-Certificate.

6) e-Receipt and e-Certificate will be provided about 3 weeks after the course finished and the submission of Online Evaluation. The link for Online Evaluation Form will be permanently removed about 3 weeks later. Should you wish to apply / re-apply the e-Receipt and e-Certificate after the deadline, an administrative fee of HKD 100.00 is required.

7)   Quiz : Doctors are required to complete a quiz after the completion of each lecture.

1) CME/CNE Accreditation is in application.

2) Cancellation /Withdrawn: No cancellation orwithdrawn will be allowed once you have submitted your application. Course Fees are NOT REFUNDABLE,except in the event of the course being oversubscribed or cancelled.

3) The Federation of Medical Societies of Hong Kong (FMSHK) reserves the right to cancel or re-schedule the course due to any unforeseen circumstances.  The FMSHK is not responsible for any cost incurred by the collaborating organizations or the participants, should cancellation or any changes take place after any participants registered for the course.
