As an organization, we recognize that humans are growing and learning and with that comes missteps. This form serves as a way for anyone to report a concern with the organization as a whole and/or any of the humans in our community. If you've experienced harm as a result of our policies, practices, or procedures, know that we want to know and do better and appreciate your feedback! If you're concerned with the behavior of someone trained by AMHC, we'd like the opportunity to address that so we appreciate your feedback!
