REQUEST FOR ADDRESS CHANGE

Please fill this out if you have had an address change since you have ordered. If you do not have a confirmation of submission please check to make sure it went through. Please also make sure to put in a forward *just in case* :)

An address change is NOT guaranteed. I take no responsibility for making sure this is done, sometimes they get over looked, do not go through and are not there when I ship, or are submitted too late for me to change because your label is already printed/shipped. In the end it is the BUYERS RESPONSIBILITY to make sure they have a forward in from their old address to their new one through usps. This is a courtesy service, not a mandatory thing for me to do. Please be AWARE of this when you request an address change please. Thank you. If your package goes to your old address I AM NOT RESPONSIBLE.

This will help find the transaction if paid by credit card and not PayPal. If not noted, your address may not be able to be changed and your order will be shipped to your old address. Please do not put your transaction ID here. The number will only be 4-5 numbers and is called your order number or invoice number and is found on your shop receipt. If you do not include it, your order will not have it's address changed. If you have multiple rounds needing an address change, this will need to be filled out for each round separately - if you put them in one submission one/or none may be changed.  IF YOU DO NOT INCLUDE THIS YOUR ADDRESS CHANGE WILL BE SKIPPED******
