In consideration of being permitted to enter the play area and/or participate in any party, activity, or program at Let’s Move & Glow Silent Disco, the undersigned, on behalf of themselves and the participant(s) listed below, acknowledges, understands, and agrees to the following:
1. Authority to Sign
I represent that I am the parent or legal guardian of the participant(s) named below, or that I have obtained permission from the parent or legal guardian to execute this agreement on their behalf.
2. Compliance With Rules
I agree that the participant(s) named below and I will follow all posted safety signs, written policies, rules, and verbal instructions provided by Let’s Move & Glow Silent Disco staff. If I observe any unsafe condition or hazard, I will immediately notify the nearest employee or official.
3. Assumption of Risk
I understand that participation in Let’s Move & Glow Silent Disco activities, programs, parties, and use of the play areas involves inherent risks. On behalf of myself and the participant(s) named below, I knowingly and voluntarily assume all such risks—both known and unknown—including those arising from the ordinary negligence of other participants.
4. Release of Liability
For myself, the participant(s) named below, and our respective heirs, assigns, administrators, personal representatives, and next of kin, I hereby release, waive, and hold harmless Let’s Move & Glow Silent Disco, its owners, affiliates, officers, members, agents, employees, other participants, and sponsoring organizations from any and all claims, injuries, liabilities, or damages arising out of or related to our participation in any Let’s Move & Glow Silent Disco activity, program, party, or use of the play areas.
Video, Photo & Media Release
I grant Let’s Move & Glow Silent Disco the right and permission to use any photographs, video, or other media captured on its premises. This media may be used for any purpose, including but not limited to printed materials, electronic publications, websites, advertising, and promotional content.
Acknowledgment & Acceptance
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT. I FULLY UNDERSTAND ITS TERMS AND ACKNOWLEDGE THAT BY SIGNING AND SUBMITTING THIS FORM, I AM VOLUNTARILY WAIVING CERTAIN LEGAL RIGHTS. I SIGN AND SUBMIT THIS FORM FREELY AND WITHOUT INDUCEMENT.
