The purpose of this form is to enable Fayson Lakes Association members to request that a new maintenance item be added to the maintenance backlog for Board of Trustees review, discussion, and prioritization.

The Maintenance Trustee will generally provide initial acknowledgment of this request as soon as possible of it being submitted, and will follow-up with you after your request has been reviewed by the Board of Trustees.
