Please fill out this form indicating what size and quantities of your Level 6 Regional Qualifier athletes. The Regionals protocols for this year is that Coaches will be responsible for picking up leos all at once for the team Athletes and Parents will NOT be allowed to pick up leos.  

When you are entering leo orders in form you will not do it by athlete name, but rather just the number you need per size.

3) Gym Contact Phone #
4) Gym Contact Email Address
5) Amount of each size that you will need

PLEASE MAKE SURE THAT THE COACH PICKING UP THE LEOS AT REGIONALS KNOWS WHAT SIZE EACH ATHLETE NEEDS.
