In an effort to assure the best possible pet adoption, we ask that you complete this application. We reserve the right to refuse any adoption that we feel is not in the best interest of this animal. A refusal does not represent any shortcoming of the applicant, but reflects our assessment that a particular pet would possibly not do well in the circumstances offered.

Please make sure you and your family are ready for the expense, work, and commitment needed to ensure a successful ownership for the lifetime of the cat. While we try to pre-screen medical and temperament issues before placing an animal for adoption, some behaviors or conditions can go undetected . We cannot guarantee health or temperament and cannot be held liable for medical expenses after adoption has been finalized.

I have read the application's information carefully and have provided answers honestly. I understand that omission of information and/or failure or refusal to answer all questions and sign therein may result in this application being declined. If an omission or untruth is discovered after an animal is placed in my care, the animal's sponsor reserves the right to annul the adoption and reclaim the animal. I give the animal's sponsor permission to fully investigate the information provided and contact any and all references listed herein. If the application passes review, I agree to provide an animal carrier (or a cardboard carrier will be provided upon pick up) to meet the sponsor at the Casual Cat Café at a mutually agreed upon time and date to finalize the adoption and pick up the cat. If for any reason the adoption does not work out you agree to contact the rescue to make arrangements on returning the cat. You agree to return all original vet paper work with the cat, if the adoption does not work out.

Release & Indemnification/No Liability: Adopter hereby releases Sponsor from any and all liability for personal injury, property damage, legal fees, or veterinary care, or any other expense or liability incurred by Adopter as a result of this adoption. Adopter agrees to indemnify Casual Cat Charities from any and all such claims and to pay, without limitation, any costs related to such injury, damage, or liability, including, in the case of litigation, any attorneys fees incurred by Casual Cat Charities in its defense.

Entire Agreement/Modification/Binding Effect: This Agreement is the entire agreement between you and us, and supersedes any prior understandings between you and us with respect to the subject matter of this agreement. No modification of this Agreement will be valid unless in writing duly signed by both you and us.
