Ramapo College of New Jersey Teacher Education & School Leadership Programs: Action Plan for Support Form
Purpose:

This Action Plan for Support form is to serve as documentation that a plan is in place for supporting and monitoring a teacher/administrator candidate that has been referred to the Assistant Dean of Teacher Education due to an academic or non-academic concern.

Process:

1- Before an action plan can be developed, the party with a concern must complete the RCNJ EPP Student Referral Form (https://tinyurl.com/yyrbhpf9); the referrer will be contacted by the Assistant Dean of Teacher Education or their designee within five (5) business days;

2- The Assistant Dean or the individual directly responsible for supporting and monitoring the candidate must complete this form after a meeting is held with the candidate; during this meeting, the concern/s is/are discussed with the candidate and an action plan is developed;

3- The Assistant Dean and those parties involved in supporting and/or monitoring the candidate's progress toward established goals will receive an electronic copy of the completed action plan; all recipients must acknowledge receipt of the document through a reply email as soon as possible.

Appeals Hearing:

If, after a Student Referral Form has been submitted, an Action Plan for Support has been completed, and the Action Plan has been duly followed through to the completion of the recommended Goal/s and Evaluation/s, concerns with regards to the issue/s in question are still present, either the candidate or an involved party may request an Appeals Hearing. The Appeals Hearing will convene all necessary and relevant individuals in order to arrive at an informed decision through discussion regarding the issue/s of concern. The results of the Appeals Hearing shall serve as the final judgment with regards to the issue/s in question.
