Organizational Membership Application
This membership application form is open to any organization that has 25 members or more. Membership year begins January 1st and ends December 31st. All applications will be reviewed and approved by the WSA Board. You will receive a confirmation e-mail once your application has been approved.
Membership Type
You are filling out the form for the following membership

Organizational Member with 25 members or more: Any local, regional, or sport specific sport organization that offers competitive and or intro sport opportunities for people with spinal chord injuries or other physical disabilities.
(Cost: More than 25 members $350)

Organizational Member Application
Aside from filling out this application, you must send the Executive Director at wsa1@telus.net your current Bylaws, a current list of officers, current list of members and a current financial statement.
Payment
At the end of this form, you will be prompted a window that will show you a paypal link. This link will allow you to select your membership type on our paypal account and pay online. If you do not wish to pay online, contact our Executive Director at wsa1@telus.net or by phone at 780-427-8699.

Once your payment has been sent, we will send your application for approval by our Board of Directors. You will receive a confirmation e-mail from us regarding your application and if it was successful.
