Welcome to Parkwood Farms Therapy Center, Inc. These online forms need to be completed prior to your or your loved one's first session. Please read all the information completely. Fill out what applies to you and/or your loved one and submit the forms. Copies of any medical documentation is acceptable. We have tried to keep the amount of paperwork to a minimum, but these forms are required by governing agencies to insure the safety of you and your family.  

Our center is constantly evolving, so please excuse any chaos and be understanding about any mess that may be a part of working equine facility. We are always looking to make improvements and we utilize our volunteer teams to make it happen. We appreciate your patience and grace with us as we continue to make improvements and grow our programs.
