Driver Eligibility Certificate (DEC) Request Form for Sgt. Eugene Ashley High School
Please do not request this form until you are ready to get your permit (you have your appointment with the DMV set and you've been studying the NCDMV handbook).  If you have completed the classroom AND driving portion of driver's education you may request this form.  We need up to a business week to verify information so you may pick up the DEC the following Monday if your request is submitted prior to the Friday preceding that Monday.  Only one DEC per semester will be issued, and the form is valid for 30 days.  

A parent or guardian is required to sign the form before the form can be given to students.  Parents may come to the school's main entrance/office to sign and retrieve the DEC on Monday between 9-13(1pm).  If Monday is a holiday then the following business/school day we will treat it like a Monday.  Email kyle.hodnett@nhcs.net if you have any questions.  Please note that we will not email or call you when it is ready, it takes a week to process it, and we will not fulfill your last second plea.
