Community Supported Agriculture (CSA) Application - 2015

    About the CSA:

    How Does It Work? Patchwork City Farms (PCF) with a preseason payment, will provide a selection of the freshest assortment of produce based on seasonality, production and availability. CSA members will receive approximately 1/2 bushel of food per week which amounts to 5-8 different vegetables/fruits per pickup. Payment is due in full with submission of registration form and contract. If you send in a payment after membership is full we will give you the choice to refund your payment 100% or hold the payment for the first available space. What Vegetables Do I Get? What is available will vary according to the season. Some of the produce we will have available are: Kale, Carrots, Herbs, Arugula, Potatoes, Heirloom Tomatoes, Baby Greens, Beets, Peppers, Swiss Chard, Onion, Eggplant, Sprouts, Radishes, Peas, Spinach, Winter Squash, Strawberries, Okra, Melons. How Do We Pickup?: We will give each enrolled member a punch card with the appropriate weeks that will be presented at each pick up. We are not responsible for reminding our members about pick-ups. If you cannot pick up your share, you may designate a friend, co-worker, neighbor or other person to come pick up your share. *Pick up will begin May 3, 2015 and last until November 15, 2015.

    EBT/SNAP CSA Program!!!

    Patchwork City Farms (PCF) is a member of the South West Atlanta Growers Cooperative (SWAG Co-op). SWAG Co-op in partnership with Wholesome Wave, a non-profit that works to increase availability of fresh produce to low income customers, is doubling the value of EBT/SNAP benefits. What does that mean for YOU? CSAs that are purchased with EBT/SNAP benefits are HALF PRICE!!!! Whole share is only $225 for 18 pickups and half share is only $125 for 9 pickups. The payment will be charged in $50 dollar increments with the last charge at $25. Full share - $50/month for 4 months and 5th month $12.50 Half share - $50/month for 2 months and 3rd month $12.50 PCF will contact you with payment processing instructions.

    Member Agreement

    I would like to be a member, and understand that I will share in the risk with the Patchwork City Farms & I agree to excuse them for the seasonal mishaps that might occur due unforeseen circumstances. I understand that some weeks will not have as much produce available due to the season. I understand that each week's produce is based upon seasonal availability, weather, etc. I understand that I am committing to a subscription and that I will pay in advance. I understand that my fees help cover the annual costs of labor, soil amendments, seed, equipment, etc. to develop Patchwork City Farms and that no refunds are available, however, I can cancel at any time.
