This form allows families to request an amendment to your child's sports program. These amendments can be requested from families with students from Years 3 and above. If you have any questions about this process please contact our Head of Sport - Luke Martin - lmartin@ccsc.nsw.edu.au.

Please note that we do our best to fulfil each request within a timely fashion.

Not all requests can be fulfilled as we need to consider the needs of your child, the College's ability to ensure that their program will be high quality and also the resources available.

If your request is successful please understand it may not be implemented immediately.
