Requested Days Off
If you are coaching more than one team, the requested days off will be applied ONLY to the team listed as the first team on this form. You MUST submit additional forms if the days off are to apply to more than one team that you are coaching. remember, if additional forms are not submitted, the days off are ONLY for the first team listed. There are no exceptions to this policy.

Each team is allowed a maximum of FOUR non-play dates (NOT four weekends). Please list IN ORDER OF PREFERENCE as these are not guaranteed. However, we will do our best based on all requests and courts provided.

Conflict examples: Timbers/Thorns November CUPs (11/3, 11/4, 11/10, 11/11, 11/17, 11/18); PYSA Rec Tournament (11/3, 11/10); Veteran's Weekend (11/10, 11/11); school events; Timbers/Thorns Academy training; tournaments; club-wide dinners/auctions; NLNC; Christmas weekends (12/22, 12/23, 12/29, 12/30); New Year's weekend (1/5, 1/6); coach has personal conflicts; etc. Please provide the NON-play dates below:
