I hereby enroll my child at Little Belfry Singers. As the guardian or parent of the child named above, I authorize the supervisors of the facility to take the necessary actions for the welfare of my child in case of an emergency. I affirm that I will not hold the Little Belfry Singers, or any of its affiliates or members, liable for any expenses incurred as a result of an accident. I will assume complete financial responsibility for any mishap. With no claim for payment, I consent to the use of his or her visual image for appropriate purposes, including but not limited to: still photography, videotape, electronic and print publications, and websites.
Concerts: For concerts, parents are required to purchase tickets for their children’s admission. Typically, each student is expected to buy two tickets, which serve as the admission fee for the event. Ticket prices may vary depending on the scale and size of the concert.
Monthly payments of $100 are due on the 1st lesson of each month.
A Registration Fee of $100 dollars will be collected at signing which includes:

- T-Shirt
- Binder and Accessories
