For the school year 2023/2024
This form is for the registration of children ages 4-10, for participation in Scott Street MB Church's Children's Ministry. Sunday School will take place Sunday mornings at from 9:45-10:15 a.m. Kids Club will take place the last Wednesday of each month from 6:15pm to 7:30pm.

Information received is confidential and is being gathered for the purposes of serving your Child while in the care of Scott Street MB Church. Any medical information collected here serves to authorize Scott Street MB Church, and its staff and volunteers, to obtain medical assistance in emergencies.

In the case of custody agreements, please submit to Ministry Personnel the proper form authorizing Parental contacts.
