Every candidate for the gospel ministry is required to submit an annual report of their status and progress (BCO 18-6). Your report is due annually at the meeting in which you were taken under care. For example, if you were received as a candidate at the February 2016 meeting, your report is due at the February meeting in 2017, and so on.

Please use this form to submit your report at least one month before the meeting at which your report will be reviewed. Your presbytery mentor will be asked to submit their report at the same time.
