Please use this form to inform your school of your student's absence from school.  (Note: This form is used to record absences, not to determine if the absence is excused.)

This form is for parent/guardian use only.  Any student who misrepresents themselves as their guardian by filling out this form are subject to suspension for forgery. Please note that attendance in school is vital for good performance.  To support your child's success in school, please limit tardy arrivals and early departures to only urgent medical appointments or other unavoidable appointments.
