Option 1: If selected as the winner of the title of Miss Alabama Volunteer, Miss Alabama Teen Volunteer, or Miss Camellia Teen Volunteer, I WILL accept the title and by signing below I confirm that I meet all eligibility requirements and confirm that I can meet all of the conditions of the titleholder responsibilities outlined in the state contract.

Option 2: If selected as the winner of the title of Miss Alabama Volunteer, Miss Alabama Teen Volunteer, or Miss Camellia Teen Volunteer, I WILL NOT accept the title and by signing below I confirm that this decision has been made after full consideration. I further acknowledge that even though I have made the decision to not accept the title this year, that I am still eligible for any available cash and in-kind scholarships that are to be awarded at the pageant. My making this selection, I totally waive any claims that I would have to be considered the respective winner of the titles award ed at the Miss Alabama/Miss Alabama Teen Volunteer pageant.

I understand that any changes to this form prior to competition must be completed in writing to the State Executive Director.

I understand that I can change my decision as long as a new form is completed and turned in at Contestant Pageant Check-In. No changes will be allowed after Check-In.
