RELEASE AND WAIVER OF LIABILITY

1. Release and Waiver of Liability: THIS RELEASE IS EXECUTED (See Date Of Form Submission)

2. The undersigned participant (Participant) is aware that participation in dog walking events and related activities may result in accident or injury and participant assumes the risks connected with participation in said activities. Participant hereby represents that participant and canine is in good health and does not suffer from any physical impairment which would affect participants ability to be involved in dog walking or related activities. Participant acknowledges and is aware that The Pack and all other venues hosting Bark for Brews sanctioned events has not and will not render any medical services including medical diagnosis of participants physical condition. In consideration of being permitted to participate, participant, for himself/herself and his/her personal representatives, heirs and next of kin, releases, waives, discharges and covenants not to sue The Pack and all other venues hosting Bark for Brews sanctioned events , its officers and members, owners and lessees of the premises, each of them their officers and employees, all referred to as releases, from all liability to and any claim or damage therefore on account of any injury to the person or property of the participant or resulting in the death of the participant whether caused by the negligence of the releasees or otherwise, while participant is participating in a dog walking event or any related activity, to include getting to and from the dog walking event. Participant agrees to indemnify the releasees and each of them from any loss, liability, damage or costs releases may incur due to the presence of participant in any scheduled dog walking event whether caused by the negligence of releasees or otherwise.

Participant agrees to adhere to the specified route & safety instructions & local traffic laws.(And additional -Notified by Route Maps & Organizer Announcements)

Bark for Brews will not release nor sell any information to outside parties, but does reserve the right to send information about upcoming events and related activities.

Participant agrees to this Liability Waiver for any and all future Bark for Brews events for the life of participation.
Participant assumes full responsibility for and risk of bodily injury, death or property damage due to negligence of releasees or otherwise while participating in a dog walking event or related activity associated with The Pack and all other venues hosting Bark for Brews sanctioned events.Participant agrees that this Release, Waiver and Indemnity Agreement is intended to be as broad and inclusive as permitted by the laws of the State of Florida and that if any portion of the Agreement is held invalid, it is agreed that the balance shall, not withstanding, continue in full force and effect.

Participant, being of lawful age, in consideration of being permitted to participate in dog walking events and related activities at or associated with The Pack, and all other venues hosting Bark for Brews sanctioned events does for himself/herself and his/her heirs, executors, administrators, and assigns hereby release and forever discharge The Pack and all other venues hosting Bark for Brews sanctioned events its officers, directors, shareholders, and employees, their heirs, administrators and executors of any and from any and every claim, demand, action, or right of action, of injury or personal injuries known or unknown, death, and/or property damage resulting from any incident which may occur as a result of participation in dog walking events or related activity associated with The Pack and all other venues hosting Bark for Brews sanctioned events, whether by negligence or not.

Participant further states that he/she has carefully read the above Release and knows the contents of the Release and signs this Release as his/her own free act.Participant further releases any and all employees, officers, directors, and shareholders from any claim whatsoever on account of rendering first aid, treatment or services rendered during his/her participation in dog walking events or related activity associated with or at The Pack and all other venues hosting Bark for Brews sanctioned events.By using our site and submitting your waiver you hereby consent to the online privacy policy, as stated. By submitting, posting or displaying any content, pictures and other materials on our social media pages, you grant to us a worldwide, non-exclusive, royalty-free license (with the right to sublicense) to use, copy, reproduce, process, adapt, modify, publish, transmit, display and distribute such content, pictures or other materials in any and all media, advertising, marketing or other distribution methods whether now known or later developed. Participant understands that there is no expiration on this waiver.

This Release contains the entire agreement between the parties to this agreement and the terms of this Release are contractual and not a mere recital.

By signing this agreement the participant agrees to follow the below rules, at all Bark for Brews events. If a participant chooses not to comply with the below rules the participant in question may be removed from the event and not invited to further Bark for Brews events. These rules have been set forth for the safety and wellbeing of all participants involved in Bark for Brews events.

Pet Rules:
All dogs must be leashed during Bark for Brews events.
Participants/Handlers/Dogs attend Bark for Brews at their own risk.
Handlers are liable for any injury or damage caused by their dog(s).
Handlers are limited to 3 dogs.
Dogs shall be licensed and vaccinated with a tag on their collar at all times.
Dogs in heat are prohibited.
Handler must be 16 years of age or older.
Children under 12 are not permitted without a responsible adult.
Handler must control their dog and attend to it at all times.
Handler is responsible for picking up and disposing of their dog's waste in proper receptacles.
No dogs under 4 months of age. Check with your veterinarian before introducing a new puppy to the group.
No aggressive dogs.
No excessive barking.

Note to Participants:
Be aware of diseases that can be transmitted from dog-to-dog contact at Bark for Brews and have your dog vaccinated BEFORE coming to the event. Example: Kennel cough, ring worm, etc.
