By submitting this form, I am requesting a transfer for my child to attend the above-referenced school in the White County School district for the 2020-2021 school year.  I fully understand that my child may only receive the requested transfer if space is available at the time this request is approved by the local school board.  
General Information:  (1) Transportation is the responsibility of parents/guardians.  Proof of residence may be required.  (2) SB 10 transfer may be denied due to lack of program alignment to the student's IEP. (3)  The principal may request that out-of-zone approval be terminated for non-compliance of rules/regulations and attendance concerns.
