2020 SRF Registration Refund Form
Hello,

Due to the current state of emergency in California, and coronavirus precautionary recommendations from health departments at the national, state, and county levels, we have decided to postpone the 2020 SRF conference.

Please fill out this form to request a refund for your conference fees. If someone else in your office was responsible for paying your registration costs, please forward this form to them. Please allow 1-2 weeks to process your request.

PayPal registrations: If you registered online via PayPal, SRF can refund your registration fees minus a $25 fee to cover transactions and processing costs.

Registration form: If you registered with a credit card or check, SRF can send you a refund check minus a $25 processing fee and any membership contribution.

Please cancel all of your hotel reservations directly with your accommodations.
