Summer Camp Registration
Register your child for Session 1 of our Summer Camp, running from June 15th to July 9th. Please complete all sections.

Price per class is $15; register for all 4 classes at discounted rate of $50. To finalize your registration, please make payment by cash or check (made payable to "Justabout Pediatric Therapy"). 

Class size will be limited to assure individual attention and maximum participation. Additionally, low enrollment can result in cancellation of camp (with refund). 

Please complete a separate registration form for each child you are enrolling.

Class 1: Summer Movement Exploration

In this program, kids will stay active and build confidence through fun, high-energy movement activities! Each week brings something new—sports skills, ninja courses, yoga, and fitness skills. This group helps improve coordination, balance, and strength while encouraging teamwork and having fun. Best for ages 4-
8.

Class 2: Food Exploration and Nutrition

Join us for hands-on food fun! Through sensory play, games, and storytelling, children will explore new foods, textures, and smells in a relaxed, no-pressure environment. This group helps build curiosity, confidence, and comfort with trying new foods. Best for ages 3-7. Contact us with questions.

Class 3: Kindergarten Readiness

This fun and engaging group helps children build the skills they need for kindergarten through crafts, play, and hands-on activities. Kids will practice recognizing letters, writing their name, using scissors, and following directions—all while building confidence and independence in a supportive environment. Suitable for kids working on k-level skills, typically ages 4-7.

Class 4: Board Games Social Group

In this program, kids will build friendships while practicing conversation, turn-taking, and teamwork through fun games and activities. This group supports confidence with peers, learning how to take turns, regulate emotions during play, and engaging in back-and-forth conversations in a relaxed, supportive setting.  Best for ages 4-8.

Camp activities may be held outdoors, weather permitting. Please plan to apply sunscreen to your child before each class and send a refillable water bottle.  *

Payment is by cash or check ("Justabout Pediatric Therapy"). No refunds will be issued for withdrawals or absences.

Parent Consent and Liability Waiver

I acknowledge that participation in camp activities involves inherent risks of injury. I voluntarily allow my child to participate and assume all associated risks. I release and hold harmless Justabout Pediatric Therapy, its owner, staff, volunteers, and affiliates from liability for injuries or damages arising from participation.

In the event of an emergency, and since the patient is a minor, I hereby give my permission and consent to Justabout Pediatric Therapy to administer emergency care as needed and hold Justabout Pediatric Therapy harmless in their act of good faith. If emergency services are activated by calling 911, Justabout Pediatric Therapy's owner, staff, and volunteers are not responsible for any costs incurred for medical transport or care. Parent agrees to provide two reliable contact numbers in case of emergency and is responsible for prompt attention to any calls while child is at camp.
