This year’s event will take place on Saturday, August 1, 2026, 11:00 AM-8:00 PM. Please complete the form below. Once you have completed the form you will be contacted with further information and instructions.

By completing this form, you acknowledge that volunteering for Jazz on the Ave may involve access to private information belonging to Avenue Community Services, partners, or sponsors, and you agree to keep all sensitive materials confidential. You also understand that the event will be photographed and recorded, and you grant permission for your image, voice, and likeness to be used in promotional media. Finally, you recognize that all volunteer roles are unpaid and that your participation is offered freely.
