If you are interested in having your child attend LSA this fall please fill out this form.  Please note that filling out this form does NOT guarantee enrollment.  This link is for enrollment for kids entering 1st to 5th grade this fall.  Kindergarten enrollment will open on April 30th and all incoming kindergarten families will receive an email from me explaining the process. If you are already enrolled at LSA, you DO NOT need to fill out this form.  You were emailed a re-enrollment form already.  If you can't find that form feel free to email me, and I will send it out to you again.
