This form is mandatory if your child needs to miss any type of performance or rehearsal.
Previously known conflicts for fall semester performances must be reported by August 7, 2026 to be considered for excusal.
Previously known conflicts for rehearsals or sectionals must be reported a minimum of two weeks before the rehearsal date to be considered for excusal. As much as possible, please make sure to reference the calendar to schedule doctor, dentist, orthodontist, therapy, and other appointments. We publish our calendar and make it available well in advance to aid in this planning.
Any absences outside of the submission windows above are excused at the discretion of the director. Please know that absences of any kind are a detriment to the success of our group, and as such have a negative impact your grade. Excused absences can be made up according to the Attendance Policy in the Instrumental Music Handbook. Unexcused absences can not be made up.
Please note that absence reasons need to be specific and clear in order to be excused. Parent contact info must be included for excusal.

For SHS Student Athletes: Because your absences and schedules for games / meets are not always known well in advance, please communicate directly with me as soon as you know your schedule. Very often, there is a way to have you participate in both activities on a given day, and I can help navigate that situation. Please fill out an absence form as soon as you know your schedule.
