Colorado Department of Public Health and Environment (CDPHE) can provide condoms and lubricants to organizations that work to prevent the spread of HIV and other sexually transmitted infections in communities across Colorado.
Agencies that intend to receive condoms and lubricants through this program must submit a plan through this form describing the populations they intend to serve and the events, sites, and venues where distribution is planned. If your plan is approved, these items will be sent to your agency free of charge and they must be distributed to individuals at no cost.
Once you receive your order you must confirm that you have received your full order by completing the attestation form within 7 days of receipt.
A CDPHE contract is not required to participate in this program. Agencies that have a CDPHE contract with a sexual risk reduction material / condom distribution activity must report aggregate material distribution on monthly progress reports.
