In order to qualify for any incentive prizes (including leaderboards), I understand that I must also email my organization's most recent budget electronically to Beth Kelley (beth@cfwesternva.org) by December 15, 2018. *
Foundation staff will review all information and determine the criteria for each leaderboard. Each organization will be notified electronically of the 2019 leaderboard criteria by February 15, 2019. Further instructions will be sent to the individual who submits this questionnaire at that time.
