BY SUBMITTING THIS FORM, I AGREE TO ABIDE BY THE PERRY YOUTH FOOTBALL BY-LAWS, ITS RULES AND REGULATIONS. FURTHERMORE, I AGREE TO ATTEND ALL INSTRUCTIONAL MEETINGS AND/OR INTERVIEWS DEEMED NECESSARY BY THE BOARD OF TRUSTEES OR COMISSIONER. YOUTH FOOTBALL MISSION STATEMENT: TO GIVE AS MANY BOYS AND GIRLS (AGES 5 YEARS OLD TO 6TH GRADE) AS POSSIBLE AN OPPORTUNITY TO PLAY THE GAME OF FOOTBALL AND CHEER IN A POSITIVE REINFORCED, ORGANIZED PROGRAM WITH ADEQUATE SUPERVISION, TRAINING AND PROTECTION. COACHING RESPONSIBILITIES         

1. TO PARTICIPATE IN ALL SUPPORT ACTIVITIES PERTAINING TO THE LEAGUE AND THEIR TEAM, INCLUDING BUT NOT LIMITED TO: EQUIPMENT HANDOUT AND TURN-IN, FIELD SET-UP, GAME DAY SUPPORT, FUNDRAISERS, MEETINGS, TRAINING.
2. TO ENSURE THAT EACH CHILD IS TAUGHT FUNDAMENTALS OF THE GAME AND TO TRAIN AND DEVELOP THE PHYSICAL FITNESS OF EACH PLAYER.
3. TO INSPIRE AND GUIDE THE BOYS AND GIRLS TO THE HIGHEST STANDARDS OF SPORTSMANSHIP, CONDUCT, AND COOPERATION, AND TO ENSURE A POSITIVE RELATIONSHIP WITH PARENTS THROUGH SOUND COMMUNICATION AND FOLLOW-THROUGH.
4. TO HAVE A WORKING KNOWLEDGE OF THE CONSTITUTION, BY-LAWS, AND RULES AND REGULATIONS.
5. TO PROVIDE A POSITIVE EXPERIENCE AND ALL OF THE ABOVE FOR EVERY PLAYER REGARDLESS OF GENDER, RACE, CREED OR ABILITY.
