UUCPA Publicity Form
Thank you for planning an event at UUCPA and helping to fulfill our mission of transforming ourselves, each other, and the world! To publicize the event, please fill out this form. The information will go to Debra, our administrative assistant, and as the manager of the publicity hub, she will send it to all of the outlets you mark below. She is also the person who should be informed if any of your information changes, so that she can send corrections. For such corrections, use this form:

Pictures of adult members and friends of the congregation may be posted without specific permission (form, which must be printed and signed, is here:
). • Avoid using the first name and last name of individuals in a photograph. • The names of adult members can be used freely on the web site. The names of children are not used. • Specific parental permission must be granted before using images of children (use permission form) • Consider using group photos rather than photos of individual children. • Ensure that the image file is appropriately named – do not use children’s names in image file names or ALT tags if published on the web • Only use images of children in suitable dress.
