Hey there, Future Foster Family!

We’re SO glad you’re here! Thank you for considering opening your heart and home to one of our senior or special needs pets. We truly believe that these animals have so much love to give, and with your help, we can give them a second chance at the life they deserve.

We want to make sure fostering is a good fit for both you and the animal, so we’ve put together this application to get to know you better. Please take your time filling it out as honestly as possible—this helps us match you with the perfect furry friend. The last thing we want is for you or the pet to feel mismatched, so being open about your lifestyle, experience, and needs will make all the difference!

Here’s what you can expect as a foster: we cover all medical costs (yep, we’ve got you covered!) and work with our trusted veterinary partner, Waterloo Animal Hospital in Edmond, OK. Fosters will need to take their pets there for routine care, but we’re here to support you with most other supplies to help your foster thrive.

We’re all about making a positive impact on the lives of these amazing animals, and we can’t wait to team up with you to do just that! I understand and agree that Silver Paws Golden Days Co. still retains full legal ownership over any animal. I understand and agree I do not have legal rights over the animals I foster for SPGD Co. I understand and agree I will not rehome, adopt, sell, giveaway, transfer, etc my SPGD Co. foster(s).  *

I understand and agree that my SPGD Co. fosters can only go to approved veterinarians for approved services by the organization. I understand and agree that I will NOT be refunded or reimbursed and will be fully responsible for any medical treatment I seek for my fosters outside of those agreed upon by SPGD Co. I understand if I choose to make a payment or donation to a veterinarian I will not be reimbursed.  *

I understand and agree that I will NOT be reimbursed for any items I buy for my foster. I understand and agree that reimbursement for items for my foster must be approved by a SPGD Co. board members BEFORE I buy it and that I must have my receipt for the item. *
I understand and agree that I cannot make medical decisions for my fosters. While my input is greatly appreciated and respected ultimately all medical decisions including euthanasia are up to the discretion of our veterinarians and board members.  *
I understand and agree that I will keep up with medical care of my foster animals which includes but is not limited to, routine and follow up appointments, vaccinations, spay/neuter procedures, heartworm/flea/tick prevention, and treatment for illnesses.  (The rescue is responsible for all financial expenses for said treatments). *
I understand and agree that it is my responsibility to communicate any medical, behavioral, or other concerns I have for the animal in a timely manner (24 hours or less depending on severity). I under and agree that it is my reasonability to act in my fosters best interest.  *
I understand and agree that I am responsible for providing transportation for my foster animals through SPGD Co. to events(if necessary), veterinarian appointments, etc. I understand and agree that if I am not able to do this that I am responsible for finding transportation accommodations for my foster. *
I understand and agree that if I decide that I am no longer able to foster an animal that I will give a minimum of at least 2 weeks notice for the RESCUE to find new accommodations. I understand and agree that I am not able to place the foster animal in a new environment/with new people without the approval of a SPGD Co. board member. I understand and agree that I am NOT going to place this animal in an animal shelter.  *
I understand and agree that all items provided/paid for by SPGD Co. will be returned to the rescue after I no longer need them for my SPGD Co. foster animal.

I understand and agree that I will provide a loving and safe home for any foster animal I have through. I understand and agree that I will provide all needed training and socialization for my foster animal. *
I understand that SPGD Co. can and may choose to remove any foster from my care without explanation or previous notice.  *

I understand that if my foster is adoptable that adoption events are a key part of finding a forever home, in order to help find my foster a home I will bring them to all adoption events when cleared for adoption or make prior arrangements for them to attend.  *
I understand and agree that I will keep SPGD Co. up to date on my foster animal(s) through them including but not limited to their health, behavior, temperament, etc. I understand and agree that I will be able to provide pictures and videos of my foster animals through SPGD Co. *
I understand and agree I will not board my SPGD Co. foster. If I defy this I understand SPGD Co. will not be responsible for any boarding fees and that I will pay them. I will communicate any needs for someone to take over my foster ASAP.  *
I acknowledge that everything that I have written, initialed, and signed in this document is TRUE and that none of the information that I have provided is FALSE. *
I understand that this is a legally binding contract and agree to all its terms and conditions.
