CLIENT LIABILITY WAIVER AND RELEASE AGREEMENT State of Hawaii IMPORTANT: THIS IS A LEGAL DOCUMENT. BY SIGNING THIS AGREEMENT, YOU ARE WAIVING CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE. PLEASE READ CAREFULLY.

1. ACKNOWLEDGMENT OF RISK
I, the undersigned, acknowledge that my participation in the use of facilities and services at Sento & Spa (hereinafter referred to as “the Facility”) involves known and unknown risks which could result in physical or emotional injury, illness, death, or damage to myself, to property, or to third parties. These risks include but are not limited to:
● Slips, trips, and falls due to wet or uneven surfaces
● Burns or overheating in saunas, steam rooms, or hot tubs
● Cold exposure in cold plunges
● Dizziness, fainting, or cardiovascular events
● Allergic reactions to cleaning products or natural materials
● Infection, skin irritation, or other health complications from shared or public facilities
● Pre-existing medical conditions aggravated by temperature changes or steam exposure
● Improper use or misuse of the facilities or equipment by myself or others
● Actions or negligence of other guests, participants, or staff I fully understand and voluntarily accept all of the risks associated with my use of the Facility.

2. MEDICAL ACKNOWLEDGMENT
I affirm that I am in good physical condition and do not suffer from any medical condition that would prevent or limit my participation in the use of the Facility. I acknowledge that Sento & Spa has not and will not provide medical advice related to my physical condition or ability to use any of the facilities or services. I understand that I should consult with a qualified medical professional before using saunas, steam rooms, hot tubs, cold plunges, or any wellness therapies, especially if I am pregnant, have cardiovascular issues, respiratory conditions, high or low blood pressure, or other health concerns

3. VOLUNTARY ASSUMPTION OF RISK
I hereby voluntarily assume all risks associated with my use of the Facility. I understand that I may withdraw from using the facilities at any time. I accept full responsibility for my own actions and any consequences thereof.

4. RELEASE OF LIABILITY
In consideration of being permitted to enter and use the Facility, I, for myself, my heirs, executors, administrators, legal representatives, and assigns, hereby release, waive, discharge, and hold harmless the following parties from any and all liability, claims, demands, actions, or causes of action, including but not limited to those for bodily injury, illness, death, property damage, or other loss:
● Sento & Spa
● Its owners, directors, officers, employees, agents, contractors, affiliates, volunteers, and successors
● The owner(s) of the premises where the Facility is located
This release includes, without limitation, any liability that may arise from negligence or carelessness on the part of the released parties, from dangerous or defective equipment or property, or from any other cause whatsoever, to the fullest extent permitted under Hawaii law.

5. INDEMNIFICATION
I agree to indemnify and hold harmless the released parties against any and all claims, liabilities, damages, or expenses (including attorney’s fees and costs) that may arise as a result of my participation or presence at the Facility, whether caused by my own actions or the actions of others

6. USE OF FACILITIES
I agree to comply with all posted rules and staff instructions. I understand that the Facility reserves the right to deny access or remove any individual for any reason, including behavior deemed unsafe, disrespectful, or in violation of Facility policies. I further understand that:
● The Facility is not responsible for lost, stolen, or damaged personal property.
● Minors must be accompanied by a legal guardian at all times and are subject to additional restrictions.
