Refund requests will be accepted up until June 12, 2020.
No refunds will be processed until after June 1.
First and last names of all students registered & refunds being requested should be listed.

This is the total you paid for your student or students. Please contact Karen Dexter at
if you are unsure.
New Registration Total
You may calculate all your registered students together. For example Gail & Kristin 2 classes each = 75x2=150 each or $300. If you are not re-registering please enter 0.
Total Refund Request
Subtract Total Amount Paid by New Registration Total to calculate your refund and enter. If you have no new registration, re-enter the Total Amount Paid.
A copy of your responses will be emailed to the address you provided.
