Opt Out - Health Curriculum
We are providing this form to families of children attending the Medford Township Public Schools to exercise their right to opt their child(ren) out of the Family Life and/or Sexual Health portion of the Health Curriculum for the current school-year. If you wish to opt multiple children out of the curriculum, you must complete this form for EACH child.

Based on New Jersey Administrative Code, N.J.S.A. § 18A:35-4.7, states, "Any child whose parent or guardian presents to the school principal a signed statement that any part of the instruction in health, family life education or sex education is in conflict with his conscience, or sincerely held moral or religious beliefs shall be excused from that portion of the course where such instruction is being given and no penalties as to credit or graduation shall result therefrom."

You will have the ability to opt your child out of the aforementioned curriculum at any time this year. There are no other areas of the curriculum, beyond the above statute, which allow for opt out provisions.
