FHS Schedule Change Conflict Form 2018-2019
This form is to be used if there is a mistake in your schedule only. Schedule change requests are only considered if there are issues due to graduation requirements, ICAP plans, credits earned via summer school or e-credit, or technical errors.

Given the complexity of the schedule and process, we will not consider requests involving teacher preference, lunch periods, free block preferences or changes of interest.

Please allow our counseling staff the summer weeks to process your request, as summer schedules fluctuate.
