Parents, Tutors of Tempe Elementary School District Students (TESD),

Please complete this form for your student to be included in the gifted testing administration for the 2025-26 school year. Requests for gifted testing are available to TESD students in all grades except K, 1st, 2nd and 4th grades; during the first semester TESD will blanket test all enrolled students in K, 1st, 2nd and 4th grade. If your TESD K, 1st, 2nd and 4th grader missed this blanket testing in the fall, please contact the school.

By completing this Request for Gifted Testing Form, the individual authorizes Tempe Elementary School District to test the named student for gifted service.

Results will be sent home with the student, 4-6 weeks after the conclusion of the testing cycle (see dates above).

Favor de completar este formulario para que su estudiante sea incluido en la administración de pruebas para Niños Superdotados/Gifted para el año escolar 2025-26. Las solicitudes de pruebas para Niños Superdotados/Gifted están disponibles para los estudiantes del TESD, en todos los grados excepto para K, 1º, 2º y 4º grado. Durante el primer periodo, TESD realizará pruebas generales a todos los estudiantes inscritos en K, 1º, 2º y 4º grado. Si por algún motivo su hijo de K, 1º, 2º y 4º grado, no realiza o no realizará la prueba general de otoño, comuníquese a la escuela.

Al completar este formulario de Solicitud de Prueba para Niños Superdotados/Gifted, usted autoriza al Distrito Escolar de Tempe, para que el estudiante sea evaluado y designado para los servicios de Superdotados/Gifted.

El Distrito Escolar de Tempe enviará a casa los resultados con el estudiante, entre 4 a 6 semanas después de haber concluido del ciclo de pruebas (consulte el cronograma de fechas a continuación).
