This is an ‘Application to Train’ google form. If you are taking a holiday break, please indicate below and the form will submit.
This form must be completed by all divers who wish to train during the June/July Holidays, June 30-July 9.

Term 3 will start 14 July

If 'Training' your request will be processed and an invoice will be generated depending on your responses to the number of training sessions selected. 
Athletes cannot train unless this form is completed and they are members of our club. This implies your July-June club membership is current.

*Missed or Cancelled sessions - Athlete's may make up a missed session, only within the holiday period.
*Invoices need to be paid by the due date
*Complete one form for each athlete
