Thank you for taking the time to report your horse-related accident and tell us about the safety equipment worn at the time. Submissions will be used for research to improve rider safety and to better understand how safety equipment works. Please note that we ask during the questionnaire for photos of both the helmet or body protector worn during your accident or for any videos or photos showing the accident itself.  These can be emailed after completing the questionnaire to the address given.
