Parents,
Each TK/Kindergarten applicant is required to submit the following evaluation completed by his/her current Preschool, Transitional Kindergarten or Kindergarten teacher. We ask that you please provide this online form to their child's teacher to fill out. Please copy and paste and provide to your child's teacher. It can also be found on our website, www.stisidore.org/admissions/how-to-apply (listed in #2)
