This form is for students with a district issued chromebook during the 2020-2021 school year that needs repaired. Please refer to the District Issued Mobile Device Guide for information on coverage and charges for district devices. This guide is located on the district website under the Instructional Technology page.
All schools and offices in School District Five of Lexington & Richland Counties are closed in observance of Winter Break. Staff will return on Monday, January 4, 2021 . Please complete this form and someone will contact you within 1-2 business days of our return date to set up an appointment if the issue cannot be resolved remotely.
