INTRODUCTION

Thank you for your interest in Rockband! Please fill out this survey and provide as much information as possible and I will get back to you via email regarding availability as soon as possible.

This is just to capture information for organizing the upcoming season.

Organizing the Rockband season is a nuanced process and requires the balancing of various information and afterschool schedules. Once all inquiries for the season are collected, I will attempt to build a band and rehearsal schedule based on age, experience, and availability.

REGISTRATION, PAYMENT & PASSWORD
Schedules permitting and after you child's band is organized and confirmed, I will issue your band parent / guardian team a PASSWORD for official registration & payment on Mark Day School's ULTRACAMP website.

The registration password is the final confirmation. This protects against unnecessary financial transactions before any band is fully organized and committed.
