Equipment will only be accepted for repair / warranty claim if this form has been completed.
The address to send your equipment to, is provided upon completion.

Please note: If you decide to buy a replacement before the warranty process has been completed, this will NOT be credited under the manufacturer’s warranty policy should warranty be approved. Repair or replacement are the sole remedies under this procedure.
