Terms and Conditions:

●       I understand that class scheduling is subject to approval by the temple/cultural center.

●       I agree to adhere to the rules and guidelines set by the temple/cultural center for instructors.

●       I grant permission to the temple/cultural center to use my name and photograph for promotional purposes related to the classes.

I,  in my full capacity as the leader of Group/Organization named below, hereby acknowledge and agree to the following terms and conditions in relation to our activities conducted on the premises of HinduSamaj-Jain Temple & Cultural Center.

I understand and acknowledge that  HinduSamaj-Jain Temple & Cultural Center has established guidelines and rules of operation for the safety and well-being of all individuals on its premises.

 I agree that it is my responsibility, as the group leader, to ensure that all activities conducted on the premises or on behalf of [Group/Organization Name] comply with the established guidelines and rules of operation of HinduSamaj-Jain Temple & Cultural Center.

 I acknowledge that any deviation from the established guidelines and rules of operation may result in increased risks and hazards to participants and attendees.

I hereby release and discharge  HinduSamaj-Jain Temple & Cultural Center, its officials, employees, and volunteers from any and all liability for any injuries, damages, or losses that may occur as a result of activities conducted on the premises or on behalf of HinduSamaj-Jain Temple & Cultural Center  that do not follow the established guidelines and rules of operation.

 I understand and agree that this waiver of liability applies to all activities conducted on the premises of HinduSamaj-Jain Temple & Cultural Center and those conducted on behalf of HinduSamaj-Jain Temple & Cultural Center, regardless of the location.

I further agree to indemnify and hold harmless HinduSamaj-Jain Temple & Cultural Center, its officials, employees, and volunteers from any claims, actions, damages, or liabilities that may arise as a result of activities conducted by [Group/Organization Name] that do not adhere to the established guidelines and rules of operation.

By filling out and signing this form, I acknowledge that I have read and understood the terms and conditions of this waiver of liability and agree to be bound by them.
