RVMS Emergency Contact Update Form

Please complete this form to request to add or remove an emergency contact for your RVMS student. This request must come from a parent/guardian of the student and must come from the email address that we have on file for you in Infinite Campus. Only one emergency contact can be added per form. If you need to add or remove multiple contacts, please click the link on the next page to submit another request. If you would like the same emergency contact to be added to students at a school other than Ranch View, please contact the Registrar at the other school directly.

**Please note - if you would like your student's sibling to sign them out from school, they must be 18 years of age or older and present a valid ID. Siblings under the age of 18 may not be designated as an emergency contact. Additionally, if the sibling is listed in Infinite Campus as a member of your household, they do not need to be added as a separate emergency contact.
