This request form is designed solely for the purpose of allowing the UCLA Music Industry Clinic (“MIC”) to make an initial evaluation of the fit of potential clients with MIC.

MIC will run from January through May 2020. If you are an independent musical artist, producer, songwriter, label owner, etc. and wish to request free legal services, please complete and submit this form.

**Completing this form does not establish an attorney-client relationship with MIC and MIC will not begin any legal work until we both decide that you will become a MIC client and you have signed an Engagement Agreement.

**Please note you and/or all members of your group must be over the age of 18, and at least one member must be based in the Los Angeles, CA metropolitan area. Unfortunately, MIC is not able to provide legal assistance to minors or to those outside of the LA area.
