Directions to the Applicant: Please send an email to the James Wood Head Coach of each sport played/managed as soon as possible. If you played both JV and Varsity levels of the sport, you only need to have the form completed by the  Varsity Head Coach. If the Head Coach is no longer available, then have the Assistant Varsity Coach at the time you played complete the form. Coaches have already been sent the link and they have the form - you just need to make the request for them to complete it by emailing them.

Coach: You may use the form for multiple players. Just use the link that was forwarded to you by Mr. Sullivan for each student. If you coached the student-athlete/manager in more than one sport, you only need to complete this form once. Please be truthful and honest in your responses.

Please note the athlete’s/manager’s application will not be processed without this document. We will need them all by Wednesday April 26th.
