If you would like your counselor to write a letter of recommendation for you, please complete this form. After you complete this form, e-mail your school counselor to request a letter of recommendation.
Note: You must request a letter of recommendation at least 4 weeks in advance. Your school counselor will not complete a form in less than 4 weeks.

Is there anything special or unusual about your circumstances which your counselor should know? Is there anything which should be clarified in order to improve your chances for admission to the school of your choice?
